Our 2010 summer season registrations are nearly complete. However there are a few placements still available, please contact us by phone at 519-855-4431 or e-mail at HEADS Administrator.

These forms must be printed and signed and mailed to the following address. Please

New Players
New Players for any Age Group MUST provide proof of age (Birth Certificate or Health Card with birth date). A photocopy must be included with mailed registrations.
